The Home Health Clinical Manager assists the Director of Home Health with oversight of agency referral and intake process, OASIS review, clinical reporting, case management, and direct care planning per MD’s orders. Partaking in ongoing planning, organizing, and evaluating all clinical processes with the Director and Executive Director to provide the most effective and efficient care while meeting quality standards. This position will assume responsibilities in the absence of the Director.

The incumbent of this role will join the team in a managerial role, which offers the ability to significantly positively impact and influence the direct quality of care and services provided to clients and patients. In addition, the Clinical Manager will help lead our team of capable, committed nursing and support staff, all with a shared mission of enriching the lives of the people and families we serve.

STABILITY – During the COVID-19 pandemic and various other uncertain times for many businesses, Country Manor Home Care nurses never had to doubt their job stability. We restructured our daily operations to provide new opportunities for our community nurses when state shutdowns prevented them from continuing in-home visits. Responsibilities:

Facilitate Personalized Plans of Care: Work closely with patients and their families to develop and implement individualized care plans that address their unique needs and preferences.

Perform Skilled Nursing Services: Provide expert nursing care, ensuring that clinical procedures and protocols are followed to maintain the highest standards of healthcare.

Supervision: Oversee and guide a team of skilled healthcare professionals, ensuring the delivery of quality care that aligns with our commitment to excellence.

Collaborate with Interdisciplinary Teams: Engage in effective communication and collaboration with, therapists, and other healthcare professionals to enhance the overall well-being of our residents.

👨‍⚕️👩‍⚕️ Responsibilities of the Home Health Care Clinical Manager:

1. Clinical: Provides administrative and clinical leadership for Home Health staff to promote the delivery of quality care. Maintains knowledge base of current standards of nursing practices, Home Health Care, Federal and State rules and regulations. Completes the discharge of Home Health Care charts following audit. Attends discharge planning meetings and clinical assessments as needed. Participates in on-call rotation as scheduled. Assists Director in overseeing active case management workloads through on-going audits and reporting review for regulatory and quality compliance. Assisting Director in coordinating on-going educational training and skills development opportunities for direct care staff.

2. Financial: Has knowledge of budget and participates in promoting appropriate utilization of resources.

3. Referrals: Assists Home Health Director in referral intake review. Confirms that all direct care summaries are completed and sent to the primary physician in a timely manner.

4. Community: Goes on community visits as necessary, or to fill in as needed.

5. Quality Improvement: Fosters a culture of continuous quality improvement. Participates in Quality Improvement projects. Maintains oversight of clinical data collection. Participates in quarterly and focus audits as requested.

7. Home Care Agencies: Reviews physician orders. Completes chart audits. Maintains software program knowledge (BrightTree). Ensures all aspects of the Health Care Reform Act, in respect to practices and billing, are being followed.

8. OASIS: Review and submissions as needed.

9. Reports as requested by upper management.

10. IDT Meeting: Assisting with IDT tracking to help monitor positive clinical outcomes and successful collaboration with Therapy services and other contract providers.

✍ Qualifications Required:

• Current state licensure in the field of practice, which must be in good standing
• Graduate of a state-accredited school of nursing
• Must have excellent interpersonal skills that promote effective communication with personnel, clients, family members, visitors, government agencies, and the general public.
• CPR Certified
• Must be able to effectively read, write, and communicate in English

➕ Qualifications Preferred:

• 2 years previous nursing experience with 1 years being in Home Health
• Previous nursing management experience desired
