Responsibilities:

The Home Health RN Referral and Assessments Manager is responsible for referrals and intake processes, authorizations compliance, OASIS review, reports, case management, and direct care planning per MD’s orders

Qualifications:

• Current state licensure in field of practice, which must be in good standing
• Graduate of a state accredited school of nursing
• Must have excellent interpersonal skills that promote effective communication with personnel, clients, family members, visitors, government agencies and the general public.
• CPR Certified
• Must be able to read, write and communicate in English

Preferred Qualifications:

• 5 years previous nursing experience with 2 years being Home Care
• Previous nursing management experience desired
