As an LPN at Country Manor, you'll:
Develop care plans and participate in the review of care plans as requested
Provide nursing services in accordance with an established care plan
Make routine resident visits to assess and document physical and emotional status; implement any required nursing interventions
Assist residents to achieve and maintain the highest level of self-care and independence
Administer and maintain records of prescribed medications and treatments
Communicate pertinent information to supervising RN, physicians, family, and/or other appropriate personnel
Accurately complete admission, weekly, follow-up, and discharge charting
Provide support, assistance, direction, and supervision to other nursing and support staff as needed
Maintain positive and effective communication and relationships with the patients and their families
Other duties as assigned
​Qualifications for an LPN:
Must hold a current license and be in good standing as a Licensed Nurse in the State of Minnesota
Must be CPR certified and have completed first aid training
Must have previous experience in nursing or other healthcare/medical fields
Able to problem-solve, think critically, and take immediate corrective action if patient well-being or care is in jeopardy
Experience with memory loss preferred, but not required
Compassionate and caring demeanor
Excellent written and verbal communication skills
Previous management or leadership experience preferred, but not required
